What Is Preservation Rhinoplasty?
Preservation rhinoplasty is based on the principle of preserving the natural tissues of the nose as much as possible. Instead of removing or restructuring large portions of bone and cartilage, this technique focuses on minimal correction, repositioning, and alignment of the existing nasal framework.
In comparison, open or structural rhinoplasty is usually based on more extensive restructuring of the nasal tissues. Dr. Swaroop Singh Gambhir has been performing rhinoplasty for the last 20 years and has been using preservation rhinoplasty, as well as hybrid rhinoplasty techniques, for many years. He is a trained and accomplished rhinoplasty surgeon.
The preservation technique is designed to reduce tissue trauma. It may also help lower the risk of revision surgery when performed on suitable candidates with the right surgical plan.
Advantages of Preservation Rhinoplasty
Preservation nose surgery involves correcting the nasal framework while maintaining the natural tissues of the nose and preserving important supporting ligaments. This approach supports a more natural-looking result and a smoother recovery for suitable patients.
This technique may help with:
- Reduced post-operative swelling
- Early recovery after surgery
- No requirement for routine internal nasal packing in many cases
- Reduced risk of bleeding
- Lower risk of revision surgery
- Fewer irregularities over the nasal dorsum
Why Choose Dr. Swaroop Singh Gambhir for Preservation Rhinoplasty?
Dr. Gambhir is known among plastic surgeon colleagues in New Delhi and Gurugram as a board-certified facial plastic surgeon with dedicated training in rhinoplasty. His experience includes cosmetic rhinoplasty, functional rhinoplasty, revision rhinoplasty, preservation rhinoplasty, structural rhinoplasty, and hybrid rhinoplasty approaches.
He understands nasal anatomy and focuses on results that look natural rather than artificial or over-operated. His belief is that rhinoplasty should enhance facial harmony in such a way that the result does not look like a “surgeon trademark look.”
Dr. Gambhir continues to read, teach, and learn recent advances in rhinoplasty. According to him, rhinoplasty is one of the most complex operations in cosmetic surgery. From the initial consultation to surgery and regular follow-up, Dr. Gambhir and his staff remain in continuous touch with the patient.
During consultation, patients are informed in detail about their concerns, surgical options, and the plan that is suitable for their needs. Dr. Gambhir gives careful attention to each patient because the nose is one of the most important features of the face.
His peers acknowledge his commitment to rhinoplasty, and he is respected as a rhinoplasty surgeon in Delhi. Dr. Gambhir specializes in techniques that can improve both breathing function and nasal shape while helping restore facial balance.
As a board-certified facial plastic surgeon, Dr. Gambhir focuses on aesthetic and reconstructive procedures of the facial region. He has dedicated training in functional and cosmetic rhinoplasty from leading facial plastic and reconstructive surgeons, including Dr. P. S. Bhandari, Dr. Vitaly from Russia, and Dr. Sureyya Seneldir from Turkey.
Patients seek his expertise from Delhi and other regions, including international locations such as Canada, the USA, and Nepal. Dr. Gambhir uses imaging software to discuss realistic goals and expectations with every patient. He has training and experience in more than 2,500 rhinoplasty surgeries.
He also has experience in crooked nose rhinoplasty, which is considered one of the most challenging forms of rhinoplasty and may require grafts and multiple surgical maneuvers to achieve the best possible correction. Dr. Gambhir closely follows his patients after surgery to ensure proper healing and satisfaction with the surgical outcome.
Are You a Good Candidate for Preservation Rhinoplasty?
Preservation rhinoplasty was initially used for men and women with small to medium dorsal humps or those who wanted to reduce the overall size of the nose. Earlier, this technique was not considered ideal for patients who had undergone previous rhinoplasty surgery or those with a deviated septum.
However, with a deeper understanding of the technique over the past few years, a hybrid approach that combines preservation and open rhinoplasty can be used in selected cases to achieve the best possible result.
Patients who want to reduce a bulbous nasal tip, correct large flaring nostrils, or straighten a crooked nose may also be considered for preservation rhinoplasty along with tip work, depending on their nasal structure and treatment goals.
In general, a good candidate for surgery is someone who is not taking blood thinners, is a non-smoker, and has realistic expectations from the procedure.

Open or Closed Rhinoplasty vs Preservation Rhinoplasty
Understanding the key difference between open, closed, and preservation rhinoplasty can help patients discuss their options more clearly. The final decision should always be made by the surgeon after evaluating the nose, breathing function, skin thickness, cartilage strength, and overall facial balance.
Open Rhinoplasty
Involves lifting the nasal skin to access the cartilage and bone.
Can include reshaping or removing cartilage and bone.
May use grafts or implants to build nasal support when required.
Allows major structural changes to be performed.
May involve more swelling and a longer recovery period.
Can give an operated look if not done properly or conservatively.
Preservation Rhinoplasty
Involves minimal removal and repositioning of bone and cartilage.
Maintains the nose’s original support structures, including ligaments.
Helps achieve a natural and subtler post-operative look.
May involve less swelling and early post-operative recovery.
Can improve functional concerns as well, depending on the case.
What to Expect During Preservation Rhinoplasty
Preservation rhinoplasty is usually performed under general anesthesia. The procedure involves injecting hemostatic fluid to help reduce bleeding, followed by access through a rim incision or an inverted V incision at the columella.
Once the nasal structures are visualized after subperichondrial and subperiosteal dissection, the surgeon proceeds according to the planned correction.
If dorsal preservation is being performed for hump removal, a piece of cartilage is removed from beneath the bony cap. The nasal bones are then osteotomized and moved downward as much as required. Cartilage may be removed from the upper area just beneath the bone or near the base over the maxillary crest.
This creates space to lower the dorsal tissues while maintaining the cartilage and bone. The top of the septal cartilage and nasal bones remain intact and untouched.
If a bulbous or bifid nasal tip is being corrected, Dr. Gambhir may use advanced suture techniques and strategically placed cartilage grafts to refine the tip. This allows the surgeon to alter the shape of the nasal tip while preserving the important alar cartilage in that area.
The length of the surgical procedure varies depending on the techniques used and the number of areas that need correction.
What to Expect After Preservation Nasal Surgery
Since preservation rhinoplasty is generally less traumatic than structural rhinoplasty, recovery may be faster. Patients may experience less swelling, bruising, and discomfort, which can help them return to office or desk work earlier.
A nasal splint is applied to protect the surgical correction and is usually removed approximately 7 to 10 days after surgery.
Patients may return to desk work in about 7 to 10 days. Strenuous activity should be avoided for 6 weeks to help prevent bleeding. Nose blowing is not allowed for 3 weeks.
Complications After Preservation Rhinoplasty
As with any rhinoplasty procedure, preservation rhinoplasty also carries possible risks. The chances may be lower in suitable cases, but patients should understand the possible complications before making an informed decision.
Possible risks include:
- Bleeding
- Bruising
- Infection
- Residual deformity
- Deviation
- Minor irregularities
- Septal collapse
- Anesthesia-related complications
Post-Operative Rhinoplasty Essentials
Begin walking on the evening of surgery to support circulation and healing. Continue walking as much as possible during the recovery period.
Avoid bumping the nose and avoid strenuous activity for 6 weeks. Do not blow your nose, and sneeze only with your mouth open. Avoid housework for 2 weeks and avoid swimming or contact sports for 4 weeks.
Preservation Rhinoplasty Recovery and Healing
Preservation rhinoplasty recovery is usually faster compared to other methods because the surgery causes minimal tissue injury. Bruising and swelling are generally minimal.
A splint is applied after the procedure and is removed in about 7 to 10 days. In the early post-operative phase, patients usually have less discomfort. Medication is provided to support healing and recovery.
Preservation Rhinoplasty Cost in New Delhi
Preservation rhinoplasty cost in New Delhi can range from Rs. 1,50,000 to Rs. 5,00,000 on average, depending on the surgeon’s experience and expertise, the geographic location, and the extent of correction required.
As the complexity of the procedure increases, the duration of surgery and the overall cost may also increase due to higher surgical facility and anesthesiologist fees.
Preservation Rhinoplasty Techniques
There are different techniques used in preservation rhinoplasty. The most suitable technique depends on the patient’s nasal anatomy, dorsal hump, septal deviation, and desired correction.
Push-Down Preservation Rhinoplasty Technique
The push-down technique is mainly used for hump removal. In structural rhinoplasty, the bony cap is removed from the top and the nasal bones are infractured. In the push-down technique, cartilage is removed from beneath the bony cap, and the bones are not touched dorsally.
Dr. Gambhir routinely performs this technique for patients who are ideal candidates.
Upper Lateral Cartilage Preservation
In this approach, the septum and upper lateral cartilage framework that forms the dorsum is not disturbed. The whole complex is moved downward as one unit, and cartilage is removed from beneath this complex.
Ligament Preservation
Important ligaments that provide structural support, such as Pitanguy and scroll ligaments, are preserved during the procedure.
Nasal Bone Management
The nasal bone is osteotomized laterally and transversely so that the whole dorsum, including the osteocartilaginous framework, can be pushed down into the nasal cavity.
Let Down Technique
The let-down technique was initially used in patients with a very high dorsal hump, especially when more than 3 mm of reduction was required. When space is limited, a residual hump may remain. To compensate for the reduction, a piece of bone may be excised from the lateral side. This is known as the let-down procedure.
The basic principle of preservation rhinoplasty remains the same: the complete osteocartilaginous framework is lowered while preserving the natural dorsum.
Dr. Gambhir understands these complexities and decides when to use push-down or let-down preservation rhinoplasty based on the patient’s nasal structure.
Instrumentation Used in Rhinoplasty
Rhinoplasty requires a basic rhinoplasty set, but finer corrections may require piezotomes or piezo instruments. Dr. Gambhir uses advanced instrumentation for precise correction during preservation rhinoplasty.
Spare Roof Preservation Rhinoplasty
In cases where there is a severely deviated septum requiring near-total correction, and the aim is to preserve the dorsum, a spare-roof technique may be used. This allows the surgeon to perform the required septal correction, including extracorporeal septoplasty when needed, while preserving the dorsal framework.
